CarriersScore — Carrier Authorization & Consent to Release

DRAFT v0.1 — for attorney review. Not legal advice.

Effective date: [EFFECTIVE DATE] · Last updated: June 7, 2026

This Authorization governs the consent a motor carrier ("Carrier," "you") gives, through the CarriersScore Service, to release its profile, full report, documents, and related information ("Carrier Information") to a requesting party in connection with a Hard Pull. It supplements the Subscriber Terms of Service and Privacy Policy. Capitalized terms have the meanings given in the Subscriber Terms.

1. What you are authorizing

When you grant consent in the Service (for example, by approving a specific requester's Hard-Pull request or enabling sharing of a document), you authorize CarriersScore to compile and disclose your Carrier Information — which may include your operating-authority and safety data, insurance status, the CarriersScore Score and sub-scores, and documents you have uploaded (e.g., certificates of insurance, W-9, authority letters) — to the specific requesting party and within the scope you select. You may grant, limit, time-bound, or decline consent.

2. Scope, duration, and revocation

3. Your representations

You represent and warrant that: (a) you are authorized to provide this consent on behalf of the Carrier; (b) the information and documents you submit are accurate and you have the right to share them; and (c) for any information that identifies an individual (including any driver, owner, or personnel data, and any motor-vehicle-record or CDL information), you have obtained all necessary consents and have a lawful basis to share it, consistent with the Driver's Privacy Protection Act (18 U.S.C. §2721 et seq.) and applicable privacy laws.

4. Snapshots and recordkeeping

A Hard Pull made under your consent creates an immutable, timestamped Snapshot recording what was disclosed. Because the Snapshot is a record of a completed transaction, CarriersScore and the requesting party may retain it as a business and compliance record even after you revoke consent or close your account, for the retention period described in the Data Sourcing, Accuracy & Dispute Policy. This recordkeeping is part of the purpose of the Service and you consent to it.

5. Driver and motor-vehicle-record data

If the Service processes any driver or motor-vehicle-record data, it does so only for DPPA-permissible purposes and/or with the express written consent of the individual, and any such data may be stored as a status/verdict with the underlying record held by a qualified vendor. You are responsible for obtaining driver consents where you submit or authorize driver data.

6. Accuracy and disputes

You may review your profile and dispute or correct information as described in Doc 07. For FMCSA-sourced fields, use FMCSA's DataQs process.

7. No fee to control your own consent

Managing your own consents and your own carrier profile does not, by itself, require a paid subscription; carriers operating twenty (20) or fewer power units may use the free carrier tier. (Requesters pay for Hard Pulls; see the Payment Policy.)

8. Electronic consent (ESIGN/UETA)

You agree that your electronic actions in the Service (clicking "I consent/authorize," approving a request, toggling sharing) constitute your electronic signature and consent, are legally binding, and satisfy any requirement that a record or signature be in writing, under the federal ESIGN Act and the Uniform Electronic Transactions Act. You consent to receive related disclosures electronically. You may request a paper copy of a consent record at info@carriersscore.com.
